BOTOX SAVINGS PROGRAM FOR SEVERE UNDERARM HYPERHIDROSIS
• Botox is a prescription medicine that is injected to help with the symptoms of severe underarm hyperhidrosis.
• Botox is covered by some insurers, but many patients still have out-of-pocket costs.
• If you are prescribed Botox, Allergan offers financial assistance programs to help cover the costs.
• Various programs exist to support patients with private or public insurance, as well as the uninsured.

Private/Commercial Insurance Plans: Depending on your insurance coverage, eligible insured patients can receive reimbursement to help with out-of-pocket costs. You may be eligible if you’re 18 years or older and have commercial insurance. Not applicable for patients whose medical insurance is provided by state or federal programs, like Medicare and Medicaid.

The Botox Savings Program covers out-of-pocket costs up to $1000 per treatment for up to 4 treatments over a 12-month period.
STEP 1 – Patients enroll in botoxsavingsprogram.com, complete their profile and download their digital savings card.
STEP 2 – Please call the number on your insurance card to confirm which specialty pharmacy they require you to use. Call us at 406-969-6829 to provide us with your pharmacy information.
STEP 3 – Our office sends the Botox prescription to the specialty pharmacy to be filled. We request that the prescription be delivered to our office at 2294 Grant Rd, Billings MT 59102 M-F 8am-4pm. The medication must be kept refrigerated upon receipt.
STEP 4 – The specialty pharmacy will collect your out-of-pocket responsibility (co-pay/co-insurance), simply provide the BIN, PCN, GRP#, and ID# from your savings card.
STEP 5 – Once the medication arrives, we can schedule treatment.

Diagnostic office visit charges and drug injection fees may still apply. The Botox Savings Program may reimburse you for the injection fee once you submit your insurance EOB (explanation of benefits). Please contact our office at 406-294-9515 with questions.
